The medal is hooked on the left upper corner of the diploma. On the obverse the is the profile of King Gustaf V of Sweden and on the reverse the symbols of the kingdom of Sweden with the caption: "Femte Olympiaden Stockholm 1912". Diploma's caption: "A Nadi, S.M. le Roi de Suède ayant daigné vous conférer Sa médaille commémorative de la V Olympiade à Stockholm 1912, j'ai l'honneur de vous remettre cette médaille, qui se porte comme d'autres décorations. Palais royal, Stockhom le 27 juillet 1912. L. CTE Douglas Grand maréchal du royaume".
